Physician Referral Form
A physican's referral is required for residents of TN to see a dietitian. To access my form, please click below or use your own. Referrals can be faxed to: (855) 239-8430

Out-Of-Network Coverage
Some insurance companies will reimburse for a portion of the visit if I am not in-network with your plan. Follow the steps outlined above under Verification of Benefits to find out. I can provide you with a Superbill to submit to your insurance company.
Self-Pay Rates
If you do not have insurance coverage for nutrition counseling, no worries! I offer self-pay and package pricing that is discounted because I believe that food is medicine and everyone should have access to nutrition counseling and medical nutrition therapy.
In-Person vs. Telehealth Visits
For your convenience I offer both in-person and telehealth appointments. All telehealth appointments take place via a secure portal in your Healthie account. Most insurance plans cover telehealth at the same rate as in-person appointments.
